Most churches (including ours) manage this by adding a note to a person's "Person Profile". Basically, just open the person's record, click on "Add a Note" and start typing, adding images, etc as required (see below). You can also add notes to a family record if a visit involved the whole family etc. All notes are automatically date-stamped and the note author is also logged. In both cases, notes can be flagged "Private" too.

The only drawback with this approach is family notes don't show in individual family members and notes for an individual won't be displayed in the family view either.
