Now that we added some issue templates, we can add a pull request template too.

Some areas to include:

Description
Related issue
Use case?
Check list for things that need to be done before we'd accept, such as:

Ensured code meets WordPress Coding Standards
All new functions and classes have code documentation
Tested with recent version of WP
No issues in ESLint
XXXXXXX
