I'm not sure this is an editor or core bug (if it's core I'm happy to report there). When I insert an image block and then go on to edit it, when I go back to the editor the old version of the image is still there - none of the edits are reflected. Closing the editor doesn't help either - when I save, exit editor, then go back to edit again the image is still the old, pre-edits version. The only thing that seemed to help, was to remove the image and insert it again from the media library. I know this sounds pretty complicated, so I made a video of the issue:
Testing with WordPress 5.7 beta 1 and Twenty Twenty One.
Editing an image in the media library as you mentioned above and adding the image back into the Gutenberg screen shows the image before it was edited.

A similar issue has been reported in #16977 but I'm not 100% sure it's the same thing.
