Build an attendee feedback form for speakers

As a speaker it's interesting to have a feedback on your talk to know how you performed and how to improve but we rarely get some.

So gospeak can have a feedback form to help gather it.
This feedback can have some of the elements below:

• a star rating (so it can be aggregated to have a global note and maybe compared with others)
• a bingo grid with affirmations to choose (such as https://twitter.com/OkazariBzh/status/979440185640448000)
• a comment area to give more qualitative and detailed feedback (with hints on what feedback to give: "what was the most interesting part?", "what did you learnt?", "what was not clear?"...)

This feedback is sent to attendees that have checkin (if enabled) just after the event (at 22h for example).
They can also access it on the page just after checkin (if enabled) so they can answer immediately after the talk.
