No Python run/debug configuration options shown

When in a python file with a main method, I choose Run -> Add configuration and this is what shows.
All the Python options are missing. I've looked in output Windows for Python Debugger and LS, but there are no errors.

Is the Python debugger extension installed? I also don't know it that menu is alphabetized (probably is, though).

Yes, latest version of ms-python.debugpy is installed. I'm debugging unit tests without any problems.

Can you open in issue using "Report Issue"? I think VS Code controls that entire list, but first step is a bug report.
