Test issue reporter UI updates

Summary
There are several small UI changes this iteration for the issue reporter. The user data sections have moved to the bottom of the page from the middle and are no longer summary elements. The inputs should now conform to the same style as other inputs in VS Code (no rounded edges, except for selects on Mac, which still use the native control throughout VSCode).

Testing
Test that the "Include x" section at the bottom works as you expect it to, and that the presentation is reasonable.
