readline: deprecate undocumented exports
This commit moves several of readline's undocumented functions into an internal module. Specifically, isFullWidthCodePoint, stripVTControlCharacters, getStringWidth, and emitKeys are moved to the internal module. The existing public exports of the first three functions are given a deprecation notice.
