Don't require editing nsfshell.s to change the credit data.

  • Move NSF title, author, and publisher strings to the score
  • Move song titles to the score
  • Add song authors (e.g. "Beethoven, arr. Yerrick"), initially written only as comments. (This will figure into a future issue.)

- required INFO, DATA, and NEND chunks
- regn if PAL compensation is enabled
- auth (preliminary, pending changes in #38)

Not done:

- psfx (length depends on sound effects count; would require
  changes to pentlyas)

- NES shell: takes track names from score, not tracknames.txt
