Please describe the problem (or idea)

What happened just before the problem occurred? Or what problem could this idea solve?

The current issue template is outdated.

What did you expect to see that you didn't?

An updated issue template by modifying the .github folder (here) and make it similar to the issue template in the plots2 repository (here).
