bug: Smart Apply Intersperses Code within the file

Describe the bug

When running apply to add generated code to a file, it often will apply in the wrong places in the file; append to the end of the file resulting in duplicate code, or intersperse it where my ide is currently looking in the file on the open tab. I don't have specifics for reproduction as it seems to be somewhat random, but seems like there could be some improvements to smart apply. It's a little too unreliable for me to use at this point versus just copy / pasting.

Expected behavior

Smart apply button correctly overwrites the correct lines in a code file.
