# WHO can gargle my ass.

(This first part is a rewritten bit of some some tweets I wrote back in March 12, 2020)

Please note... I am not a doctor, so please take this with a \*grain of salt\*.

## Have You Considered Saline?

I'm thinking if we can use effective gargling in a medical settings we might be able to prevent build up in nasal/mouth/throat before it gets enough to enter lungs and overwhelm immune system? May also help prevent/slow spread.

So gargle iodized salt?

What does the World Health Organization say?

There is no evidence a preventative measure we haven't bothered to test at scale has effect? 🤨

Let's see.

No evidence my ass.

Here's a study on saline. ¯\\_(ツ)_

Doesn't this count as evidence? I don't see how they can say with a straight face that there is \*no\* evidence any preventative measure we haven't bothered to test at scale has any effective power against "novel coronavirus". However there *is* evidence that salt water does have action against Coronavirus in general, and a low power clinical trial suggests it might work. But I'm sure that this is just a *Myth*. Sure is awfully kind of the world health organization to tell me that.

How do we... Uh... Get access to a large number of potential cases to run a clinical trial on the use of salt water gargle as a partial preventative measure against contracting the common cold? You'd think cases would be hard to find... /s

Some time later...

Now, to follow up on this idea in September 28th, 2023. Just in time to see a rise in covid cases and concerns over spreading...

However, this study found that the NaCl solution had no virucidal activity against SARS-CoV-2, even though high concentration of NaCl (0.34 M) was used. More study should be done using various NaCl concentrations to determine the effect of NaCl against SARS-CoV-2.

So that does imply that salt water is doing something to boost viral resistance and could help protect people. And more than that the overall study concluded that mouthwashing/gargle behaviors do indeed have virucidal activity. So we should probably encourage people to do that in a way that works.

And what happens if they actually try different concentrations? When this was investigated further it turns out the concentration of the salt is indeed a huge factor.

oh, and so.. uh. What happens if you rinse your nose with a hypertonic solution of salt water like the WHO was really decrying?

Well consider this article in Forbes:

Only 1.3% of study participants who used nasal irrigation were hospitalized after testing positive for Covid-19. While 9.47% of Covid patients from a national CDC dataset who did not use nasal irrigation were hospitalized.

## Bad mythbusting backfires.

While myth-busting can combat misinformation, it can also backfire when used prematurely on complex, evolving topics. The WHO's goal is presumably to provide credible guidance, but their messaging here outpaced the available science.

Promoting it even just as a safe, accessible preventive measure worth exploring further would be a more scientifically balanced approach than an outright dismissal before the science is fully settled. Public health recommendations could reflect these complexities.

Calling it a "myth" without acknowledging these broader considerations is an oversimplification by the WHO. A more nuanced perspective would be to say the evidence for salt water directly combating SARS-CoV-2 specifically is still inconclusive or preliminary, while noting its potential value in other areas.

Even if salt water gargling proved ineffective specifically against SARS-CoV-2, it could still provide value by:

* Reducing the overall circulating levels and transmission of other common respiratory viruses through its potential antiviral effects. This could lessen the burden on the healthcare system from other illnesses.

* Being a very low-risk and low-cost practice with possible additional benefits beyond direct antiviral activity, such as reducing throat inflammation.

* Allowing more research time and data to be collected on its impact (or lack thereof) on SARS-CoV-2 and new variants before making a definitive conclusion. Dismissing it prematurely may close doors to further understanding.

Instead they chose to call it a lie. This strikes me as incompetence bordering on malice.

Calling something a "myth" has dismissive connotations that go beyond the scientific evidence. Some more constructive ways the WHO could have framed this issue include:

* Stating the evidence for salt water against SARS-CoV-2 is inconclusive/preliminary rather than definitive "no evidence".

* Acknowledging potential indirect benefits even if direct effect uncertain.

* Encouraging further research before making strong conclusions either way.

By prematurely labeling it a "myth", they risked:

* Discouraging promising research avenues that could aid public health.

* Diminishing public trust if/when evidence emerges that contradicts the original stance.

* Failing to consider broader context beyond just SARS-CoV-2 specifically.

## And the damage is done.

Not sure about you, but I think its funny that [the WHO no longer has that bit on their website](https://web.archive.org/web/20220210202445/https://www.who.int/emergencies/diseases/novel-coronavirus-2019/advice-for-public/myth-busters). You can still see how many other local health ministries in local municipalities parrot the old one as they blindly copied that text still say it though.

I don't trust the World Health Organization anymore. They jumped to conclusions without evidence and then as a trusted source of information they ended up spreading what amounts to lies and misinfo.

The other one I noticed they removed was Garlic. I wonder how long until they remove the Hot Pepper one, cuz it turns out that taking hot pepper in your food may indirectly prevent coughs and could reduce the spread that way. But what do I know? I'm not a doctor.

While the WHO's aim is presumably to combat misinformation, the messaging here did not align well with the nuanced realities of an evolving scientific issue. A more cautious, evidence-based framing allowing for uncertainties could have avoided perceptions of incompetence or worse. Their current framing was and still is counterproductive and risks undermining their credibility on Covid advice going forward. A correction or clarification may be warranted.

The WHO calling salt water gargling a myth early in the pandemic likely led to that idea being dismissed or seen as debunked in many people's minds. Even though they no longer have that on their website, the myth label probably still shapes perceptions for some who aren't closely following the evolving research.

This illustrates how authoritative sources like WHO need to be very judicious and nuanced when communicating about new or uncertain scientific issues to the public. Simplistic myth busting can inadvertently entrench misconceptions if the evidence later shifts. It can undermine trust in future guidance too.
