Improve new initiative form

• Load previous initiatives from local storage
• Add questions 4i-4iii from page 26 of the technical instructions
• adjust questions about $ to be:

1. what is your ballpark estimate of TOTAL ADDITONAL cost?
2. If possible break this down into
   a. Personnel
   b. Non-personnel
3. Estimate of additional revenue (if applicable)

• Add text to clarify that this submission is the starting point for a conversation with both OB and ODFS, who will help with the details.
• Ask whether revenue is recurring or one-time
• Add question about account string
• Lower priority : allow $ and commas in the # fields, but still validate that it's numeric and check that the total = personnel + nonpersonnel (if provided)
