Fix CTAP2 tests

This is a tracking PR that contains all changes required to pass the CTAP2-test-tool:

• Fix CTAP2 tests ctap-types#7
• Fix CTAP2 tests fido-authenticator#22
• usbip: Add interactive user presence check #314
• Use a reference to the interchange data instead of copying it to the stack trussed-dev/trussed#122
• Use reference to the response rather than copying it trussed-dev/usbd-ctaphid#6

This causes nitropy nk3 test to fail with:
