Add the qiskit-ecosystem theme (#506)

This makes the `_qiskit-ecosystem` now public.

It also documents how to use it - closes
#486. Note that the
default theme should be `qiskit-ecosystem`; only Terra should use
`qiskit`.

Also fixes some broken links in the README - closes
#488.
