The feedback form is 3 different screens per user, with a lot of unnecessary options and selections.

From the Admin meeting, we think that we only need a "something went wrong", and a "I'd like to tell you something". This would reduce the UI complexity if it much more. We can keep some of the complexity if debug: True in the config, which would allow our testers to give detailed feedback, but that complexity can be reduced as well.

Should be fixed in #12, at least for now. Qualitative research might help here, but don't think it should be a priority, given we still need to see how these changes play out.
