This reverts commit ce27461.
We will never show it on the debug page (because it requires that debug is off), and needs to be asked there, otherwise we would never show it to end users.

All the answers are now on one screen, and the number of options you have to
choose is limited. Also added back some of the icons that were used in the MAVC feedback form.
Also reduced some duplicate description text for testers, and put it in more
useful locations in the interview.
Only major reduction in information is that we don't ask testers to fill in different areas for bug reports, instead encouraging them to write their bug reproduction in a single text area. IMO that shouldn't make too much of a difference, but I can bring it back if desired.
