Heres the .appveyor.yml file. Now you need to setup Appveyor. Go to ci.appveyor.com and login with github. Then click on 'add project' and authorise if required. After this select 'reload' and press 'New Build'. As long as the .appveyor.yml file was put in the root directory, everything should go fine.
