Create README.md for express, hapi and Koa #373

Currently the readmes in those packages just link to the docs. We should create proper readmes so that users can see them on npm. It's as simple as copying parts from the main README and the docs section for each server, all the text is already written. The only things to document is how to set up a simple GraphQL endpoint and GraphiQL.

Volunteers and new contributors very welcome!

@HriBB do you think you'd be able to put together the readme for Koa?
@nnance any chance someone on your team could make a PR with the readme for Hapi?

Not required by any means, but I think it would be very useful, and I don't currently have the time to do it myself.
