Fixes two issues with link replacements:
Links being replaced when the alternative is an empty string/disabled. This happens when there is a card e.g. a youtube-preview or the result is just the domain e.g. "www.wikipedia.org"
"medium.com" links are always replaced with "farside.link/scribe" ignoring the value from the environment variable
