The exynos4412 has four extra combined irq group and the exynos4212 has two more combined irqs than exynos4210. Each irq is mapped to IRQ_SPI(xx). Unfortunately, extra 4 combined IRQs isn't sequential. So, we need to map the irqs manually. This patch defines irq numbers of ARM performance monitoring unit for exynos4. Firs of all, we need to fix IRQ_PMU correctly and to split pmu initialization of exynos from plat-samsung for easily defining it. The number of CPU cores and PMU irq numbers are vary according to soc types. So, we need to identify each soc type using soc_is_xxx function and to define the pmu irqs dynamically. For example, the exynos4412 has 4 cpu cores and pmus. Avoid circular mutex lock by pushing the dev->lock to the .fini callback on each extension. As em28xx-dvb, em28xx-alsa and em28xx-rc have their own data structures, and don't touch at the common structure during .fini, only em28xx-v4l needs to be locked. This patch addresses an traditional iscsi-target fabric ack starvation issue where iscsit_allocate_cmd() -> percpu_ida_alloc_state() ends up hitting slow path percpu-ida code, because iscsit_ack_from_expstatsn() is expected to free ack'ed tags after tag allocation. This is done to take into account the tags waiting to be acknowledged and released in iscsit_ack_from_expstatsn(), but who's number are not directly limited by the CmdSN Window queue_depth being enforced by the target. So that said, this patch bumps up the pre-allocated number of per session tags to: (max(queue_depth, ISCSIT_MIN_TAGS) * 2) + ISCSIT_EXTRA_TAGS for good measure to avoid the percpu_ida_alloc_state() slow path. This patch changes percpu_ida_alloc() + callers to accept task state bitmask for prepare_to_wait() for code like target/iscsi that needs it for interruptible sleep, that is provided in a subsequent patch. This patch addresses a bug where connection reset would hang indefinately once percpu_ida_alloc() was starved for tags, due to the fact that it always assumed uninterruptible sleep mode. So now make percpu_ida_alloc() check for signal_pending_state() for making interruptible sleep optional, and convert iscsit_allocate_cmd() to set TASK_INTERRUPTIBLE for GFP_KERNEL, or TASK_RUNNING for GFP_ATOMIC. The send operation processes inodes by their ascending number, and assumes that any rename/move operation can be successfully performed (sent to the caller) once all previous inodes (those with a smaller inode number than the one we're currently processing) were processed. This is not true when an incremental send had to process an hierarchical change between 2 snapshots where the parent-children relationship between directory inodes was reversed - that is, parents became children and children became parents. When creating network portals rapidly, such as when restoring a configuration, LIO's code to reuse existing portals can return a false negative if the thread hasn't run yet and set np_thread_state to ISCSI_NP_THREAD_ACTIVE. This causes an error in the network stack when attempting to bind to the same address/port. This patch sets NP_THREAD_ACTIVE before the np is placed on g_np_list, so even if the thread hasn't run yet, iscsit_get_np will return the existing np. Also, convert np_lock -> np_mutex + hold across adding new net portal to g_np_list to prevent a race where two threads may attempt to create the same network portal, resulting in one of them failing. This patch fixes a percpu_ref_put race for se_lun->lun_ref in transport_lun_remove_cmd() where ->lun_ref could end up being put more than once per command via different target completion and fabric release contexts. It adds a cmpxchg() for se_cmd->lun_ref_active to ensure that percpu_ref_put() is only ever called once per se_cmd. This bug was manifesting itself as a LUN shutdown regression bug in >= v3.13 code, where percpu_ref_kill() would end up hanging indefinately due to the incorrect percpu_ref count. This reverts commit f2d9b99. We are ripping out commit 35773da "usb: xhci: Link TRB must not occur within a USB payload burst" because it's a hack that caused regressions in the usb-storage and userspace USB drivers that use usbfs and libusb. Solution: This patch is doing a quick fix for that, it adds spin_lock/unlock(&port->lock) protect to sirfsoc_uart_pio_rx_chars() in sirfsoc_rx_tmo_process_tl() to keep spin_lock/unlock in pair. A selective retransmission request (SRR) is a fibre-channel protocol control request which provides support for requesting retransmission of a data sequence in response to an issue such as frame loss or corruption. These events are experienced infrequently in fibre-channel based networks which makes it difficult to test and assess codepaths which handle these events. We were fortunate enough, for some definition of fortunate, to have a metro-area single-mode SAN link which, at 10 GBPS sustained load levels, would consistently generate SRR's in a SCST based target implementation using our SCST/in-kernel Qlogic target interface driver. In response to an SRR the in-kernel Qlogic target driver immediately panics resulting in a catastrophic storage failure for serviced initiators. The culprit was a debug statement in the qla_target.c file which does not verify that a pointer to the SCSI CDB is not null. The unchecked pointer dereference results in the kernel panic and resultant system failure. The other two references to the SCSI CDB by the SRR handling code use a ternary operator to verify a non-null pointer is being acted on. This patch simply adds a similar test to the implicated debug statement. This patch is a candidate for any stable kernel being maintained since it addresses a potentially catastrophic event with minimal downside. This patch fixes a bug in iscsit_get_tpg_from_np() where the tpg->tpg_state sanity check was looking for TPG_STATE_FREE, instead of != TPG_STATE_ACTIVE. The latter is expected during a normal TPG shutdown once the tpg_state goes into TPG_STATE_INACTIVE in order to reject any new incoming login attempts. There are a handful of uses of list_empty() for cmd->i_conn_node within iser-target code that expect to return false once a cmd has been removed from the per connect list. This patch changes all uses of list_del -> list_del_init in order to ensure that list_empty() returns false as expected. This patch addresses a couple of different hug shutdown issues related to wait_event() + isert_conn->state. First, it changes isert_conn->conn_wait + isert_conn->conn_wait_comp_err from waitqueues to completions, and sets ISER_CONN_TERMINATING from within isert_disconnect_work(). Second, it splits isert_free_conn() into isert_wait_conn() that is called earlier in iscsit_close_connection() to ensure that all outstanding commands have completed before continuing. Finally, it breaks isert_cq_comp_err() into seperate TX / RX related code, and adds logic in isert_cq_rx_comp_err() to wait for outstanding commands to complete before setting ISER_CONN_DOWN and calling complete(&isert_conn->conn_wait_comp_err). This patch fixes the incorrect setting of ->post_send_buf_count related to RDMA WRITEs + READs where isert_rdma_rw->send_wr_num was not being taken into account. This includes incrementing ->post_send_buf_count within isert_put_datain() + isert_get_dataout(), decrementing within __isert_send_completion() + isert_response_completion(), and clearing wr->send_wr_num within isert_completion_rdma_read() This is necessary because even though IB_SEND_SIGNALED is not set for RDMA WRITEs + READs, during a QP failure event the work requests will be returned with exception status from the TX completion queue. This patch changes IB_WR_FAST_REG_MR + IB_WR_LOCAL_INV related work requests to include a ISER_FRWR_LI_WRID value in order to signal isert_cq_tx_work() that these requests should be ignored. This patch addresses a number of active I/O shutdown issues related to isert_cmd descriptors being leaked that are part of a completion interrupt coalescing batch. This includes adding logic in isert_cq_tx_comp_err() to drain any associated tx_desc->comp_llnode_batch, as well as isert_cq_drain_comp_llist() to drain any associated isert_conn->conn_comp_llist. Also, set tx_desc->llnode_active in isert_init_send_wr() in order to determine when work requests need to be skipped in isert_cq_tx_work() exception path code. Finally, update isert_init_send_wr() to only allow interrupt coalescing when ISER_CONN_UP. The following pattern is currently not well supported by RCU: 1. Make data element inaccessible to RCU readers. 2. Do work that probably lasts for more than one grace period. 3. Do something to make sure RCU readers in flight before #1 above have completed. Here are some things that could currently be done: a. Do a synchronize_rcu() unconditionally at either #1 or #3 above. This works, but imposes needless work and latency. b. Post an RCU callback at #1 above that does a wakeup, then wait for the wakeup at #3. This works well, but likely results in an extra unneeded grace period. Open-coding this is also a bit more semi-tricky code than would be good. This commit therefore adds get_state_synchronize_rcu() and cond_synchronize_rcu() APIs. Call get_state_synchronize_rcu() at #1 above and pass its return value to cond_synchronize_rcu() at #3 above. This results in a call to synchronize_rcu() if no grace period has elapsed between #1 and #3, but requires only a load, comparison, and memory barrier if a full grace period did elapse. I'm transitioning maintainership of the xHCI driver to my colleague, Mathias Nyman. The xHCI driver is in good shape, and it's time for me to move on to the next shiny thing. :) There's a few known outstanding bugs that we have plans for how to fix: 1. Clear Halt issue that means some USB scanners fail after one scan 2. TD fragment issue that means USB ethernet scatter-gather doesn't work 3. xHCI command queue issues that cause the driver to die when a USB device doesn't respond to a Set Address control transfer when another command is outstanding. 4. USB port power off for Haswell-ULT is a complete disaster. Mathias is putting the finishing touches on a fix for #3, which will make it much easier to craft a solution for #1. Dan William has an ACKed RFC for hardkernel#4 that may land in 3.16, after much testing. I'm working with Mathias to come up with an architectural solution for #2. I don't foresee very many big features coming down the pipe for USB (which is part of the reason it's a good time to change now). SSIC is mostly a hardware-level change (perhaps with some PHY drivers needed), USB 3.1 is again mostly a hardware-level change with some software engineering to communicate the speed increase to the device drivers, add new device descriptor parsing to lsusb, but definitely nothing as big as USB 3.0 was. The Synopsys APB DW UART has a couple of special features that are not in the System C model. In 3.8, the 8250_dw driver didn't really use these features, but from 3.9 onwards, the 8250_dw driver has become incompatible with our model. This patch changes isert_conn_create_fastreg_pool() to follow logic in iscsi_target_locate_portal() for determining how many FRMR descriptors to allocate based upon the number of possible per-session command slots that are available. This addresses an OOPs in isert_reg_rdma() where due to the use of ISCSI_DEF_XMIT_CMDS_MAX could end up returning a bogus fast_reg_descriptor when the number of active tags exceeded the original hardcoded max. This patch fixes a bug where outstanding RDMA_READs with WRITE_PENDING status require an extra target_put_sess_cmd() in isert_put_cmd() code when called from isert_cq_tx_comp_err() + isert_cq_drain_comp_llist() context during session shutdown. The extra kref PUT is required so that transport_generic_free_cmd() invokes the last target_put_sess_cmd() -> target_release_cmd_kref(), which will complete(&se_cmd->cmd_wait_comp) the outstanding se_cmd descriptor with WRITE_PENDING status, and awake the completion in target_wait_for_sess_cmds() to invoke TFO->release_cmd(). The bug was manifesting itself in target_wait_for_sess_cmds() where a se_cmd descriptor with WRITE_PENDING status would end up sleeping indefinately. This patch fixes a long-standing bug in iscsit_build_conn_drop_async_message() where during ERL=2 connection recovery, a bogus conn_p pointer could end up being used to send the ISCSI_OP_ASYNC_EVENT + DROPPING_CONNECTION notifying the initiator that cmd->logout_cid has failed. The bug was manifesting itself as an OOPs in iscsit_allocate_cmd() with a bogus conn_p pointer in iscsit_build_conn_drop_async_message(). With EXT4FS_DEBUG ext4_count_free_clusters() will call ext4_read_block_bitmap() without s_group_info initialized, so we need to initialize multi-block allocator before. And dependencies that must be solved, to allow this: - multi-block allocator needs in group descriptors - need to install s_op before initializing multi-block allocator, because in ext4_mb_init_backend() new inode is created. - initialize number of group desc blocks (s_gdb_count) otherwise number of clusters returned by ext4_free_clusters_after_init() is not correct. There was a very small race window where resume to kernel mode from a Exception Path (or pure kernel mode which is true for most of ARC exceptions anyways), was not disabling interrupts in restore_regs, clobbering the exception regs Anton found the culprit call flow (after many sleepless nights) | 1. we got a Trap from user land | 2. started to service it. | 3. While doing some stuff on user-land memory (I think it is padzero()), | we got a DataTlbMiss | 4. On return from it we are taking "resume_kernel_mode" path | 5. As trace event triggers are now part of the mainline kernel, I added my trace event trigger tests to my test suite I run on all my kernels. Now these tests get run under different config options, and one of those options is CONFIG_PROVE_RCU, which checks under lockdep that the rcu locking primitives are being used correctly. During the recent conversion of cgroup to kernfs, cgroup_tree_mutex which nests above both the kernfs s_active protection and cgroup_mutex is added to synchronize cgroup file type operations as cgroup_mutex needed to be grabbed from some file operations and thus can't be put above s_active protection. After 96d365e ("cgroup: make css_set_lock a rwsem and rename it to css_set_rwsem"), css task iterators requires sleepable context as it may block on css_set_rwsem. In fact, given the fact that a cgroup may contain large number of tasks, it's not a good idea to iterate over them while holding IRQ-safe spinlock. Let's simplify locking by replacing per-cgroup freezer->lock with global freezer_mutex. This also makes the comments explaining the intricacies of policy inheritance and the locking around it as the states are protected by a common mutex. The conversion is mostly straight-forward. The followings are worth mentioning. * freezer_css_online() no longer needs double locking. * freezer_attach() now performs propagation simply while holding freezer_mutex. update_if_frozen() race no longer exists and the comment is removed. * freezer_fork() now tests whether the task is in root cgroup using the new task_css_is_root() without doing rcu_read_lock/unlock(). If not, it grabs freezer_mutex and performs the operation. * freezer_read() and freezer_change_state() grab freezer_mutex across the whole operation and pin the css while iterating so that each descendant processing happens in sleepable context. This patch fixes a free-after-use regression in ft_free_cmd(), where ft_sess_put() is called with cmd->sess after percpu_ida_free() has already released the tag. Fix this bug by saving the ft_sess pointer ahead of percpu_ida_free(), and pass it directly to ft_sess_put(). end_offset and req_offset both return u64 - avoid casting to u32 until it's needed, when it's less than the (u32) size returned by nfs_generic_pg_test. Also, fix the comments in pnfs_generic_pg_test. If an inode is renamed, it should be registered as file_lost_pino to conduct checkpoint at f2fs_sync_file. Otherwise, the inode cannot be recovered due to no dent_mark in the following scenario. Note that, this scenario is from xfstests/322. 1. create "a" 2. fsync "a" 3. rename "a" to "b" 4. fsync "b" 5. Sudden power-cut After recovery is done, "b" should be seen. However, the result shows "a", since the recovery procedure does not enter recover_dentry due to no dent_mark. The reason is like below. - The nid of "a" is checkpointed during #2, f2fs_sync_file. - The inode page for "b" produced by #3 is written without dent_mark by sync_node_pages. So, this patch fixes this bug by assinging file_lost_pino to the "a"'s inode. Sharvil noticed with the posix timer_settime interface, using the CLOCK_REALTIME_ALARM or CLOCK_BOOTTIME_ALARM clockid, if the users tried to specify a relative time timer, it would incorrectly be treated as absolute regardless of the state of the flags argument. This patch corrects this, properly checking the absolute/relative flag, as well as adds further error checking that no invalid flag bits are set. As the netpoll controller method doesn't block real napi polls in any way, there is a potential for race conditions in which the netpoll controller method and the napi poll method run concurrently.
