feat: quiz in classes

Class

• Moderators can now add quizzes to class.
• They can add and remove quizzes from the dialog that opens on click of Manage Assessments.
• Students can visit the quiz submission page from there when they open their progress page.
• From there they can attempt the quiz.
• With each attempt, they see the score from all previous attempts
• When a student has attempted the quiz, the moderator can verify the score from the student's progress page.

Quiz

• Moderators can now set Max Attempts for a quiz from the LMS Portal.
• Once the number of attempts exceeds the max attempt, the student will no longer be able to attempt the quiz again.
