Fixed parameter metadata issue with packaged store procs

- Fixed an error with Oracle thin JDBC driver when retrieving parameter
metadata for a stored procedure inside a package.
- Cleaned up compilation warnings
- Correctly parse stored procedure call when procedure name is identity
quoted.
