add fields to the Biotope History section of biotope description page

Currently the biotope description pages have a section near the bottom called Biotope History, which shows previous versions of the biotope code. This is populated with the OLD_CODE and VERSION columns of the WEB_OLD_CODE table in the db. With upcoming changes to biotopes, we need a more comprehensive summary of the history of each biotope that includes the full name, and describes what changes have been made. The WEB_OLD_CODE table from the website's master database has already had a few extra columns added (PREVIOUS_FULLNAME, RELATIONSHIP_TYPE, MODIFICATIONS), but they aren't yet filled in. We'll work on that in the coming months. Once the new biotopes are published (maybe early 2020), it would be good to make it so these extra fields show up in the table on the biotope description page.

Also, change the heading 'Biotope history' to 'Revision history'.
