It would be nice to revert this when AMD fixes the issue though. Just need to keep track of that somewhere.

Use ordinal_type to index over grid points and rows, and size_type to index over matrix entries. This applies to all backends, but on HIP it also works around a compiler bug with division-with-remainder.

In structured matrix explicit construction, be more consistent with integer types. Use
ordinal_typefor rows, nodes and grid points, and usesize_typeto index over matrix entries.This also works around a bug in hipcc for release builds. When you compute both a/b and a%b (division with remainder), where a is size_t and b is int, the quotient comes out wrong. This broke the structured spmv tests when size_type = size_t and ordinal_type = int.
