Fixes #3220.

Driven-by changes:

Also `_STD`-qualify all _Ugly names in `<scoped_allocator>` and `<xpolymorphic_allocator.h>`.

Also change try-catch block in `<xpolymorphic_allocator.h>` to scoped guard.
