Fix low-res mac icons (#3223)

Addresses issue #2404. Something about how our Mac .icns files were created caused the low-res icons to display incorrectly in macOS. Strangely, all resolutions display properly in all of the .icns viewing applications I tried, including Preview. However, I was able to repro the problematic macOS icon displaying behavior by using iconutil to extract out the packaged PNGs in the icns. In the future, we should verify any new macOS .icns icons that we add to the application using iconutils.
