BREAKING CHANGE: Node v12.20, v14.14, or v16.0 are the new minimum requirements

Removes the esm package and switches to using native Node ES modules.

This is probably a breaking change for self-hosters, because we'll have to bump the minimum required Node version to whichever ones support ESM.
