Using index-based lookups relies on JPhyloRef returning phylorefs in a particular order, which it doesn't.

The embargoed phyloreferences included in this PR were curated by Rebecca Stubbs (@stubbsrl) from June to August 2018.
Do not review: I'd like to double-check all this before submitting it for review. Also, we don't yet have a way of storing the curator of a phyloreference (phyloref/klados#59) -- it might be worth holding on to this PR for a few days so I can make that happen first.
