interpret: support for per-byte provenance #104054

Also factors the provenance map into its own module.

The third commit does the same for the init mask. I can move it in a separate PR if you prefer.

add provenance-related test

One of the cases in #2182 actually works as intended since rust-lang/rust#104054 (or maybe even earlier); make sure we test that.
