Hi, I've had the same problem. Well kinda. As soon as I moved far enough (in game) for new regions to render, my game would turn invisible, so instead of minecraft there was a blank screen. I figured out it had to do with optifine and thanks to your comment and trial and error I could find the cause for this problem, which is
Render regions: ON
you need to turn render regions off for the problem to be solved.

Smart animations: ON
I managed to reproduce it only once when i was testing to see what the issue was, it is either one of the two or both causing the crash for me
