User Experience improved while adding/removing a question

Mainly User Experience improvement and a minor bug fix

1. #5 DeleteAll button is present and can be used to remove all notifications/questions.
2. #4 Show a confirmation dialog when user clicks performs watch/unwatch operation.
3. #13 Show confirmation dialog before deleting a notification/question.
4. [Bug fix] Icon is not present on question pages when URL contains '#'
