Add domain consensus hashing

- Added domain hashing to all previous uses of default consensus hashing.
- Changed `output_mr`, `witness_mr `kernel_mr` and `input_mr` types in
  `pub struct BlockHeader` to be `FixedHash` instead of variable length
  `BlockHash`.
