This PR fixes the issue zfcampus/zf-apigility-admin#304 for the UI showing an input text for the table name in case of db autodiscover failure. I tested using the ibmdb2 adapter, that doesn't support the autodiscover feature at the moment.
Moreover, the PR contains some fixes for the database options in the REST page.
Important: this PR should be merged after the PR zfcampus/zf-apigility-admin#305
