Remove Ubuntu 16.04 as no longer available #1173

Remove the link to Ubuntu 16.04 Windows Store item as it has been removed. I've left the direct link to the appx package as is still working, though I'm not sure I'd recommend 16.04 at this point as it will be EOL as of tomorrow. Can remove as part of this PR if people deem necessary.

Resolves issue #1172

Version 1803 has been end of life, no annotations required.

* replace old GCM with GCM Core (#1188)

The old Git Credential Manager for Windows is deprecated, having been replaced by the cross-platform GCM Core.

I also fixed up a few points of confusion between "Git" and "GitHub", and fixed capitalization on "Git" where needed. ("Git" is the software project; "`git`" is the command-line binary.)
