  1. Clean up ConfiguredTargetValueAccessor and ConfiguredTargetAccessor 

    - Fix looking up a target from the walkable graph.
  - This prevents ConfiguredTargetValueAccessor from needing to look into
    ConfiguredTargetAccessor.
  - And removes duplicated code.
- Move CTVA into the correct package.
