Model update

• Updates models with the fancy new @immutable fields courtesy of our friends at Ceramic.
• Indexes are added back after the upstream indexing bug has been fixed
• Require all revocation fields to avoid issues from them being null in state, so we can always filter on true/false
• Remove now-deprecated template data generation
• Add Model ID's and track stability in documentation
• Models have been deployed to the Clay testnet, and the package should now work to interact with them independent of which ceramic network is targeted (given that they have been deployed, of course).
