I see no real difference in the changes. What exactly was wrong and how can we prevent this in the future?

Minified plugin's sourcemaps where from the previous version (v6.5.1). This happens when I don't manually launch the build (gulp build) before deploying. I'll investigate this. But for now I just need the up-to-date sourcemaps to be released.
